The Certified Occupational Therapy Assistant (COTA) at Serenity Oaks IRF works under the supervision of a licensed Occupational Therapist to deliver occupational therapy interventions and assist patients in achieving ADL independence, functional cognition, and meaningful occupational performance. The COTA implements OT treatment plans, monitors patient response, and maintains accurate documentation under OT supervision. This role is an essential contributor to the high-intensity therapy program at Serenity Oaks and supports patient progress toward rehabilitation goals and safe discharge.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES
- Implement occupational therapy treatment plans as established by the supervising OT.
- Deliver ADL retraining, upper extremity exercises, cognitive activities, and adaptive equipment training as directed.
- Monitor patient response to treatment and report changes or concerns to the supervising OT promptly.
- Assist with FIM self-care scoring as directed and supervised by the OT.
- Maintain accurate and timely daily treatment notes in the EHR per facility requirements.
- Assist patients with self-care training, adaptive techniques, and therapeutic activities.
- Provide patient and family education on ADL strategies, equipment use, and home safety as directed by the OT.
- Set up and maintain therapy equipment and adaptive devices; ensure a safe therapy environment.
- Communicate patient progress and concerns to the supervising OT.
- Participate in IDT meetings as directed by the OT or Rehabilitation Program Director.
- Participate in CARF program evaluation and facility education activities.
- Adhere to AOTA COTA standards and Louisiana COTA scope of practice regulations.
EDUCATION & EXPERIENCE REQUIREMENTS
- Required: Associate degree in Occupational Therapy Assisting from an ACOTE-accredited program.
- Required: Successful completion of NBCOT COTA examination.
- Preferred: One (1) or more years of COTA experience in inpatient rehabilitation or acute care.
LICENSURE, CERTIFICATION & REGISTRATION REQUIREMENTS
- Required: Current, unrestricted Occupational Therapy Assistant license issued through the Louisiana State Board of Medical Examiners (LSBME).
- Required: Current NBCOT COTA certification.
- Required: Current BLS/CPR certification (AHA or Red Cross).
- Required: Louisiana COTA license verified through LSBME prior to and throughout employment.
KNOWLEDGE, SKILLS & ABILITIES
- Knowledge of occupational therapy treatment techniques for rehabilitation populations.
- Ability to implement OT plans of care under OT supervision within COTA scope.
- Understanding of FIM self-care scoring in COTA context.
- Knowledge of ADL retraining and adaptive equipment use.
- Proficiency with EHR daily note documentation.
- Effective communication with supervising OT, patients, and families.
