Position Summary
We are seeking an experienced Nurse Practitioner (NP) to provide mobile pain management services to our patients in AL/SNF environments. The ideal provider is clinically strong, confident managing complex geriatric patients, and committed to timely documentation and clear facility orders.
Key Responsibilities
- Perform pain-focused assessments for acute and chronic pain conditions
- Develop individualized treatment plans emphasizing:
- safe prescribing practices (non-opioid first, evidence-based approaches)
- monitoring for adverse effects (falls, sedation, constipation)
- functional improvement and care coordination
- Manage medication regimens, including controlled substances when clinically appropriate
- Coordinate with facility staff for medication orders, safety monitoring, and follow-up
- Review labs and imaging as appropriate; refer to specialty care when indicated
- Provide clear written orders and complete documentation on time
Required Qualifications
- Active, unrestricted Arizona NP license
- National certification (AANP or ANCC)
- DEA license (required if prescribing controlled substances)
- 2+ years experience in pain management, internal medicine, geriatrics, LTC/SNF, ortho, or related
- Ability to travel to facilities across Mesa and surrounding areas
- Strong clinical judgment and excellent communication skills
- Must maintain malpractice/professional liability insurance (or ability to obtain)
Preferred Experience
- Experience working in AL/SNF/mobile care
- Familiarity with Medicare and AHCCCS workflows
- Comfort with polypharmacy review and medication reconciliation
- Experience coordinating PT/OT, imaging, and referrals
Contract Expectations (1099)
- Provider must maintain licensure, DEA, and malpractice coverage
- Documentation completion expected within 48 business hours of encounter
- Compliance with facility and MGT MD at Home workflows and communication protocols
Compensation
Competitive, encounter-based or tiered model depending on volume and facility needs. Growth opportunities as patient panels expand.
Pay: From $100,000.00 per year
