Position Summary
This position provides case management in the form care coordination, assessment of needs and determination for eligible services. In addition, this position prepares treatment plans, assists with goal setting and documents work in database. The Medical Social Assistant works under direct supervision of Medical Social Consultant.
Duties & Responsibilities
- Under direct supervision, completes on-going assessment of consumer needs.
- Develops a Treatment Plan and provides linkages and referrals as indicated by assessment.
- Follows up to determine referral outcomes and explores alternate resources as necessary.
- Maintains strict compliance with HIPAA, confidentiality and other UICOM-P and Positive Health Solutions policies and procedures.
- Identifies consumer, family, and /or environmental strengths, weaknesses, and/or dysfunction and, in consultation with supervisor and appropriate staff members, develops strategies to assist consumer in meeting goals of a mutually agreeable care plan.
- Keeps accurate consumer records according to funding source guidelines and assists supervisor in data collection to complete program reports.
- Develops awareness of, and builds relationships with support services available to HIV+ and PWAs, including but not limited to various in-home professional health care, nutrition, substance abuse treatment, housing, transportation, and other available services.
- Collaborates with Part B and others identified in the provision of case management services for targeted area.
- Consults with hospital discharge planners and other health or mental health providers to ensure continuity of consumer care as appropriate.
- Attends and contributes to department meetings, program meetings, and interagency meetings as requested by Medical Social Supervisor.
- Participates in case reviews and utilizes quality assurance instruments.
- Assists in data gathering and grant preparation as requested by supervisor.
- Perform other related duties and participate in special projects as assigned.
Minimum Qualifications
- Bachelor's degree with a major in the social or behavioral science or related health specialty from an accredited college or university.
Preferred Qualifications
- Valid Illinois Driver’s License.
- Experience with Medicaid and Medicare programs.
