As a member of the Social Services Department you will work in a fast paced environment and enjoy collaborating with internal and external sources to create a calm and confident return home for our patients.
To succeed in this position you will be required to have:

- Excellent organization skills,
- Attention to detail
- Compassion for and desire to work with people of all ages and cultures
-Nursing home experience required
Discharge planning experience preferred or other healthcare experience favored.
