REPORTING: The LVN Charge Nurse reports to the Director of Nursing Services.
JOB DUTIES:

Writes nursing assignments for nursing personnel in his/her assigned unit consistent with their education, preparation and experience and supervises as needed.

Maintains a current resident care plan for each resident on the assigned unit and monitors and updates as needed.

Assists with orientation and staff development.

Assists Director of Nursing Services and/or Nursing Supervisor with evaluation of personnel progress and need for improvement.

Participates in meetings and conferences as requested.

Assures that established policies and procedures are followed.

Maintains the resident health record with accurate and complete documentation.

Makes daily resident rounds on residents and evaluates resident needs and effectiveness of nursing care provided.

Transcribes physician orders accurately and promptly.

Monitors environment and equipment for safety and reports repairs needed.

Maintains and accurate and safe system of control of all medications including controlled drugs. Ensures that all medications are available and drug-ordering records are accurate.

Administers medications and treatments as ordered.

Promotes and practices good restorative nursing to help residents attain their highest level of function possible.

Review each resident’s orders with physician monthly.

Accompanies physicians on their visits to residents as needed.

Informs the physician, Director of Nursing Services and Responsible party immediately of significant changes in resident’s condition.

Receives and gives appropriate, accurate and concise reports at change of shift.

Responsible for general operation of the facility after administrative hours and notification of appropriate department head and Administrator should an emergency occur.

Understands and acts according to Federal, State and Local Regulations for skilled nursing facilities.

Maintains good rapport and open communication with his/her staff, physicians, families and staff of other departments and the community at large.

Assures every resident is properly identified by an armband or other acceptable method.

If unable to be on duty, as assigned, will report at least four hours in advance, except in emergencies.

Observes the facility dress code.

Observes Resident Rights at all times.

Practices and Promotes safe work habits.

Maintains acceptable standards of nursing practice and is in good standing with the licensing board.

All other duties as assigned by the Director of Nursing Services and Administrator.
REQUIREMENTS:
- Graduate of an accredited school of Nursing.

Possesses a valid and current California LVN License.
-
1-2 years experience in long term care preferred.
-
Good physical and mental health.
-
Must be able to perform any physical requirement of the position which includes lifting dead weight or unassisted weight of 40 pounds.

Is capable of following oral and written instructions.

Have training, knowledge and experience which allows for general supervision of responsibilities.
-
IV Certification preferred.
