Monarch Concierge MD is seeking a compassionate, highly skilled Nurse Practitioner to join our growing concierge internal medicine practice in Wilmington, Delaware. This is an outstanding opportunity for a provider who values meaningful patient relationships, clinical excellence, and practicing medicine without the constraints of high-volume healthcare.Our concierge model allows providers the time and resources to deliver personalized, proactive care while collaborating closely with an experienced physician and an exceptional clinical team.
What You'll Do
- Conduct comprehensive patient evaluations and follow-up visits Diagnose and manage acute and chronic medical conditions Develop individualized treatment plans based on evidence-based medicine Order and interpret laboratory tests, imaging, and diagnostic studies Prescribe medications and manage treatment plans in accordance with Delaware regulations Perform annual wellness visits and preventive health assessments Educate patients on disease prevention, nutrition, wellness, and lifestyle modification Coordinate specialty referrals and collaborate with consultants Respond to patient questions through phone, secure messaging, and telehealth as appropriate Maintain accurate and timely documentation within the electronic medical record Participate in care coordination and population health initiatives Help create an exceptional patient experience that exceeds expectations
Qualifications
- Current Nurse Practitioner license in Delaware (or eligibility for licensure) Board Certification as Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP). DEA registration and prescriptive authority. Minimum of 2 years of primary care, internal medicine, family medicine, or concierge medicine experience preferred. Experience or a strong clinical interest in functional medicine, regenerative medicine, hormone optimization, medical weight management, and other longevity-focused therapies is strongly preferred. Excellent clinical judgment and diagnostic skills Strong interpersonal and communication abilities. Passion for preventive medicine and whole-person care .Comfortable using electronic medical records and technology. Professional, polished, and patient-focused demeanor.
