RN / Case Manager
Location: Residential Facility - Brooklyn, NY
Schedule: Monday–Friday, 8:00 AM–4:00 PM
Position Type: Long-Term Temporary
Caseload: Approximately 19 residents, with direct responsibility for approximately 10 residents at a time
Position Overview
Seeking an RN/Case Manager to provide nursing and case management support within a residential setting. The role combines clinical responsibilities, case planning, documentation, staff training, and interdisciplinary collaboration.
Key Responsibilities
- Manage case plans and maintain detailed resident documentation and paperwork.
- Provide nursing support and respond to resident/staff calls and concerns.
- Conduct staff meetings, training, and in-service education for Direct Support Professionals (DSPs).
- Collaborate with staff and interdisciplinary teams regarding resident care and needs.
- Administer medications and injections when DSPs are unavailable, as permitted by licensure and facility protocols.
- Monitor resident needs and coordinate appropriate services and follow-up.
- Maintain accurate records and ensure compliance with required documentation.
Qualifications
- Current Registered Nurse (RN) license.
- Case management and/or residential healthcare experience preferred.
- Experience working with OPWDD (Office for People With Developmental Disabilities) populations is highly preferred.
- Strong documentation, communication, organization, and time-management skills.
- Ability to train staff and work effectively with interdisciplinary teams.

Pay: $49.00 - $54.00 per hour
