Mercy Drive Inc. is seeking a compassionate, organized, and motivated Bilingual (English/Spanish) Caseworker to join our growing Day Habilitation Program. This position is responsible for coordinating services, supporting participants and their families, maintaining regulatory compliance, and ensuring that each individual receives high-quality, person-centered services.

Responsibilities
- Serve as the primary point of contact for assigned Day Habilitation participants and their families.
- Coordinate and monitor services in accordance with each participant's Individualized Service Plan (ISP) and Life Plan.
- Conduct regular participant visits and maintain ongoing communication with families, Care Managers, and interdisciplinary team members.
- Maintain accurate, timely, and compliant documentation in accordance with OPWDD and agency requirements.
- Complete progress notes, monthly summaries, incident follow-up, and other required documentation.
- Coordinate admissions, transfers, discharges, and annual ISP meetings.
- Advocate for participants to ensure they receive appropriate services and supports.
- Collaborate with Day Habilitation Supervisors, Direct Support Professionals (DSPs), clinicians, and other service providers.
- Assist with regulatory audits and quality assurance activities.
- Ensure confidentiality and compliance with HIPAA and agency policies.
- Perform other duties as assigned.
Qualifications
- Associate's Degree with relevant experience may be considered.
- Minimum of one (1) year of experience working with individuals with intellectual and developmental disabilities preferred.
- Must be fluent in both English and Spanish (spoken and written).
- Knowledge of OPWDD regulations and Day Habilitation services preferred.
- Strong organizational, communication, and problem-solving skills.
- Proficient in Microsoft Office and electronic documentation systems.
- Valid New York State Driver's License and reliable transportation preferred.
