Position Overview
We are seeking a reliable, compassionate, and experienced Physical Therapist (PT) to provide in-home physical therapy services to patients throughout the Ventura and Oxnard areas.
The Physical Therapist will evaluate patients in their homes, develop individualized treatment plans, provide skilled therapy services, monitor patient progress, and coordinate care with physicians, nurses, therapists, and other members of the home health team.
We are looking for clinicians who are independent, organized, responsive, and committed to completing documentation accurately and on time.
Responsibilities
- Perform comprehensive in-home physical therapy evaluations
- Complete Start of Care-related therapy evaluations as assigned
- Establish individualized therapy goals and Plans of Care
- Provide skilled physical therapy treatments in the patient’s home
- Perform gait, balance, strength, mobility, transfer, and functional assessments
- Develop therapeutic exercise and home exercise programs
- Provide fall-prevention and home-safety education
- Evaluate patients for appropriate assistive devices and durable medical equipment
- Educate patients, family members, and caregivers
- Monitor patient progress and modify treatment plans when clinically appropriate
- Complete reassessments, recertification evaluations, and discharge evaluations as required
- Communicate changes in patient condition to the appropriate clinical team
- Coordinate care with RNs, LVNs, PTAs, OTs, STs, physicians, and other healthcare providers
- Review and follow physician orders and the established Plan of Care
- Maintain accurate visit information, including required time-in/time-out documentation
- Complete all clinical documentation accurately and within agency-required timelines
- Respond promptly to documentation corrections or QA requests
- Maintain communication with the office regarding scheduling, patient status, missed visits, hospitalizations, and changes in condition
- Follow all applicable agency policies and federal and California home-health requirements
Qualifications
Required:
- Current and unrestricted California Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Valid California driver’s license
- Reliable transportation
- Current automobile insurance
- Ability to travel to patients’ homes throughout the assigned territory
- Strong clinical assessment and documentation skills
- Ability to manage an independent field schedule
- Strong communication and time-management skills
Preferred:
- Previous Home Health Agency experience
- Experience with Medicare-certified home health
- Familiarity with home-health EMR/documentation systems
- Experience working with geriatric and medically complex patients
- Knowledge of home-health Plans of Care, physician orders, reassessments, recertifications, and discharges
- Experience coordinating with PTAs and interdisciplinary clinical teams

Typical Patient Care
Our Physical Therapists may work with patients requiring skilled therapy following:
- Hospitalization
- Orthopedic surgery
- Joint replacement
- Falls or mobility decline
- Stroke or neurological conditions
- Generalized weakness and deconditioning
- Gait and balance impairment
- Chronic medical conditions
- Functional decline
- Difficulty transferring or ambulating safely within the home
The primary goal is to help patients restore mobility, improve strength and function, reduce fall risk, and safely remain in their homes.
Territory
We are currently seeking Physical Therapists who can cover:
VenturaOxnard
Availability to cover surrounding areas within Ventura County is a plus.
Territories and patient assignments can be coordinated based on clinician availability and geographic preference whenever possible.
Compensation
Competitive per-visit compensation based on experience, visit type, territory, and availability.
Compensation structure and visit expectations will be discussed during the interview process.
