About the Role
Spirit Homecare & Hospice is seeking a compassionate and skilled Registered Nurse (RN) to provide patient-centered nursing care in patients’ homes. This position allows you to deliver high-quality care while maintaining flexibility and working independently.

Key Responsibilities
As a field-based RN, you will provide high-quality nursing care while collaborating with the care team. Responsibilities include:
- Provide skilled nursing care following physician orders and established care plans
- Conduct assessments, including OASIS, to track patient progress
- Administer medications, wound care, and other prescribed treatments safely
- Educate patients and caregivers on medications, disease management, and home care
- Collaborate with therapists, social workers, and other care team members
- Document care and assessments accurately using EMR
- Participate in on-call rotations for evenings, weekends, and holidays as scheduled
Qualifications
- Active Registered Nurse (RN) license in Ohio (Compact license a plus)
- 1–2 years of nursing experience preferred (Home Health experience highly valued)
- Familiarity with EMR systems and ICD-10 coding preferred
- Strong clinical judgment, communication, and organizational skills
- Current CPR certification
- Reliable transportation, valid driver’s license, and active auto insurance
Work Environment & Expectations
- Work Setting: Patients’ homes
- Physical Requirements: Ability to walk, lift, bend, and stand for extended periods
- On-Call Availability: Optional rotations for evenings, weekends, and holidays
- Communication: Must promptly notify the Director of Nursing of any schedule changes
