Hybrid one day in office- Nursing opportunity to support teams at all sites. Responsibilities will include:

Monitoring Provider fax boxes, triaging and responding to patients as needed.

Overseeing medication refills for an assigned patient population.

Tele heath Nurse follow up visits as assessed.

Focus on patient education, quality outcomes and overall patient satisfaction.

Experience/Education: Graduate of accredited school of Nursing, BSN strongly preferred, Demonstrated proficiency with EHR, with EPIC a strong plus.
The ideal candidate will also have worked in an outpatient setting and experience working with underserved populations.
