Position Summary:
Provides planning and delivery of both direct and indirect patient care through the comprehensive nursing process of Assessment, Planning, Intervention, and Evaluation. Develops individualized nursing care plans in coordination with patients, families, and interdisciplinary staff as appropriate. Communicates clinically significant changes in patient condition to Physicians, Nursing Supervisor/Manager, and team members as needed. Engages in and supports the discharge planning process to help ensure safe and effective transitions of care.
Pay:
Rate of pay is based on years of experience and qualifications.
