Responsibilities:
Conduct initial and quarterly assessments of residents to identify their social, emotional, and psychological needs
Develop and implement individualized care plans to address residents' needs and goals
Provide counseling and support to residents and their families to promote their physical, emotional, and psychological well-being
Collaborate with other members of the healthcare team to coordinate resident care and services
Identify and report changes in residents' behavior, conditions, or needs to the interdisciplinary team
Maintain accurate and detailed records of resident assessments, care plans, and services provided
Participate in orientation and training programs to maintain current knowledge of social services and resident care
