We're looking for a compassionate and motivated Registered Nurse (RN) Case Manager to join our Albuquerque team. This is an excellent opportunity for an RN who enjoys building relationships, coordinating care, mentoring staff, and making a lasting impact in the lives of veterans.
Unlike hospital nursing, this role allows you to spend meaningful time with your clients while helping them navigate services, coordinate care, and improve their quality of life.
What You'll Do
As an RN Case Manager, you will:
- Manage a caseload of veterans receiving home care services.
- Conduct Start of Care assessments for new clients within your assigned geographic area.
- Develop and update individualized care plans.
- Coordinate care with veterans, caregivers, VA staff, physicians, and family members.
- Monitor client progress and identify changes in condition or service needs.
- Provide caregiver education and guidance to ensure care plans are followed.
- Help veterans connect with available VA and community resources.
- Assist with onboarding and in-office training of new employees.
- Maintain accurate and timely documentation.
- Travel throughout the Albuquerque and surrounding area to complete client visits.
Qualifications
Required
- Current New Mexico Registered Nurse (RN) license in good standing.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong communication, organizational, and problem-solving skills.
- Ability to work independently while managing multiple priorities.
Preferred
- Home Health experience
- Case Management experience
- Geriatric or Veteran care experience
