Position Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing and rehabilitation setting. The LPN works closely with Registered Nurses, physicians, and interdisciplinary team members to ensure residents receive quality care in accordance with facility policies and state regulations.
Primary Responsibilities
- Provide direct nursing care to residents including medication administration and treatments
- Monitor residents’ health status and report changes to the Registered Nurse or physician
- Assist with care planning and documentation in the medical record
- Supervise and support nursing assistants to ensure quality resident care
- Ensure compliance with infection control, safety, and regulatory guidelines
- Maintain accurate and timely documentation of care provided
- Communicate effectively with residents, families, and healthcare team members
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing
- Experience in long-term care or skilled nursing preferred but not required
- Strong communication and teamwork skills
- Compassionate, dependable, and resident-focused attitude
- Ability to work in a fast-paced healthcare environment

Pay: $27.24 - $33.16 per hour
