EXAMPLE OF DUTIES
This role supports public health by assessing needs, planning programs, and guiding health activities across the Northwest District. The work helps strengthen the delivery of essential services and ensures strong connections with local partners. As a Community Health Nurse, you will perform the following duties: Health Assessments: Evaluate county level information to identify public health needs and available resourcesCommunity Partnerships: Link agency programs with community activities to support local health goalsProgram Development: Create public health initiatives based on community data and established guidelinesDisease Monitoring: Review information systems to track health programs and support improved outcomesHealth Education: Provide and develop education programs on public health topics to increase community awarenessPatient Support: Offer clinical services and case management to promote patient health and safetyInterested in learning more? Additional details regarding this position can be found in the position description.Work Schedule and Additional Information: Full-time employmentWork hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.Work infrequently requires statewide travel and irregular/flexible work hours including overnights, weekends, and holidays in accordance with policy.Telework: You will not have the option to telework in this position.Salary: In some cases, the starting salary may be non-negotiable.You will receive further communication regarding this position via email.
