Join a five-star, multilingual home care and hospice agency serving Mandarin, Cantonese, Vietnamese, Korean, and English-speaking families across the Bay Area. We're looking for a compassionate Registered Dietitian to support nutrition care for our home health and hospice patients.
WHAT YOU'LL DO
- Conduct in-home and telehealth nutrition assessments for home health and hospice patients
- Develop individualized nutrition care plans in coordination with RNs, physicians, and caregivers
- Provide patient and family education on therapeutic diets, tube feeding, and disease-specific nutrition needs
- Document assessments and progress notes in compliance with Medicare/Medicaid regulations
- Collaborate with our multilingual clinical team to serve patients in their preferred language
WHAT WE'RE LOOKING FOR
- Registered Dietitian (RD/RDN) credential, active and in good standing
- California state licensure (or eligibility)
- Experience in home health, hospice, or geriatric nutrition preferred
- Bilingual candidates encouraged to apply (Mandarin, Cantonese, or Vietnamese a plus)
- Reliable transportation for in-home visits across the service area
