Licensed Practical Nurse (LPN) – Overnight (11:00 PM – 7:00 AM)
Position Summary
The Overnight Licensed Practical Nurse (LPN) is responsible for providing nursing oversight, medication management, resident assessments, and clinical support during overnight hours while promoting resident safety, dignity, and quality of life. The LPN works collaboratively with caregivers, medication technicians, providers, families, and the Wellness Director to ensure residents receive compassionate, person-centered care in accordance with state regulations, physician orders, and facility policies.
Essential Responsibilities
Clinical Care
- Monitor residents for changes in condition.
- Respond promptly to resident emergencies and initiate appropriate interventions.
- Complete monitoring following falls, illnesses, injuries, or other significant events.
- Notify physicians, responsible parties, and emergency services as appropriate.
- Provide nursing treatments within the LPN scope of practice.
- Monitor residents returning from the emergency department or hospital for changes in condition.
- Ensure physician orders are implemented accurately.
Medication Management
- Administer medications and treatments safely and accurately according to physician orders.
- Monitor residents for medication effectiveness and adverse reactions.
- Complete narcotic counts and controlled substance documentation.
- Address medication concerns or omissions that occur during the overnight shift.
- Ensure medication carts remain organized, secure, and compliant with regulations.
Resident Safety
- Conduct routine rounds throughout the community.
- Monitor residents identified as high fall risk or requiring increased observation.
- Assist with emergency situations including falls, behavioral concerns, and acute illnesses.
- Promote infection prevention and control practices.
- Ensure emergency equipment is operational and readily available.
Documentation
- Document interventions, medication administration, and resident responses accurately and timely.
- Complete incident reports and required clinical documentation.
- Record changes in condition and communicate pertinent information to the day nursing team.
- Maintain accurate shift reports and communication logs.
Leadership and Team Support
- Provide clinical guidance and supervision to Resident Assistants, CNAs, and Medication Technicians during the overnight shift.
- Assist direct care staff with resident care as needed.
- Ensure staff follow facility policies and resident care plans.
- Report staffing concerns or urgent operational issues to the on-call manager.
Communication
- Communicate effectively with residents, families, physicians, pharmacies, and interdisciplinary team members.
- Participate in shift report and provide detailed handoff to incoming staff.
- Notify the Wellness Director or Administrator of significant resident or facility concerns.
Regulatory Compliance
- Maintain compliance with Rhode Island Assisted Living regulations and facility policies.
- Maintain resident confidentiality in accordance with HIPAA.
- Participate in quality improvement and infection control initiatives.
- Maintain current licensure and required certifications.
Qualifications
- Current Licensed Practical Nurse (LPN) license in Rhode Island in good standing.
- Current CPR certification required.
- Minimum one year nursing experience preferred; assisted living, geriatrics, dementia care, or long-term care experience preferred.
- Strong assessment and critical thinking skills.
- Excellent communication and documentation abilities.
- Ability to work independently while making sound clinical decisions.
- Proficient with electronic health records (PointClickCare experience preferred).
Physical Requirements
- Ability to stand and walk for extended periods.
- Ability to lift, transfer, push, or pull up to 50 pounds with or without reasonable accommodation.
- Ability to respond quickly to emergencies.
- Ability to bend, stoop, reach, and assist residents with mobility as needed.
Schedule
- Overnight Shift: 11:00 PM – 7:00 AM
- Weekends and holidays as scheduled.
- Participation in emergency call-back when necessary.

Pay: $33.00 - $36.00 per hour
