We are seeking someone who is a compassionate, dedicated, and caring professional to join our team!
Responsibilities:
- Provides advanced nursing care and treatment to hospice patients in consultation with and under the direct supervision of the hospice Medical Director
- In collaboration with the IDT, patient and family, provides consultation, assessments and evaluation of the plan of care
- Manage patient needs and respond to caregiver concerns as needed, to ensure comfort and care
- Counsels the patient and family in meeting nursing and related needs
- Provides health care instructions to the patient as appropriate per assessment and plan
- Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver
Qualifications:
- Graduate of an accredited Nurse Practitioner program from a recognized university and obtained a Masters Degree in Nursing
- Current licensure in SC, CPR certification and valid driver’s license and proof of insurance
- Minimum of three years nursing experience required. Prefer two years’ experience in hospice, palliative care, or geriatric setting
- Must have reliable transportation for necessary travel throughout the service area
