Wolfeboro Bay Center is seeking a compassionate, experienced, and resident-focused Director of Social Services to join our leadership team.
This is an excellent opportunity for a social services professional who is passionate about advocating for residents, supporting families, and helping individuals navigate the challenges associated with short-term rehabilitation and long-term care.
As Director of Social Services, you will play an essential role in ensuring our residents' psychosocial, emotional, and discharge-planning needs are met while working closely with nursing, therapy, physicians, families, and the interdisciplinary team.
$60,000 – $80,000 per year, based on experience and qualifications.
Lead and oversee the Social Services Department
Assess residents' psychosocial, emotional, and social needs
Develop and implement individualized plans to address identified needs
Advocate for residents and promote dignity, independence, choice, and quality of life
Provide support and guidance to residents and families throughout their stay
Participate in resident care planning and interdisciplinary team meetings
Coordinate discharge planning and ensure safe, appropriate transitions of care
Connect residents and families with community resources and services
Assist residents and families with advance directives and other care-related planning
Address resident and family concerns promptly and professionally
Support residents experiencing adjustment difficulties, behavioral concerns, grief, or other psychosocial challenges
Maintain accurate and timely documentation in accordance with facility, state, and federal requirements
Participate in quality improvement and regulatory readiness initiatives
Collaborate with nursing, therapy, admissions, activities, dietary, and other departments to provide comprehensive resident-centered care
Previous social services experience in a skilled nursing, rehabilitation, long-term care, or healthcare setting strongly preferred
Previous leadership or supervisory experience preferred
Knowledge of federal and state long-term care regulations
Strong understanding of discharge planning and community resources
Excellent communication, interpersonal, and organizational skills
Ability to effectively communicate with residents, families, healthcare professionals, and community partners
Strong problem-solving and conflict-resolution abilities
Compassionate, professional, and resident-centered approach
Ability to manage multiple priorities in a fast-paced healthcare environment
