JAG Healthcare Shelby is a welcoming 45-bed skilled nursing and rehabilitation facility with a small, family-oriented atmosphere. Our Social Service Designee plays an important role in promoting each resident’s physical, emotional, mental, and psychosocial well-being while serving as a resource for families and a liaison to community partners.

Responsibilities
(include but are not limited to):
- Coordinate resident admissions and discharge planning to promote smooth transitions of care
- Assess and support residents’ social, emotional, mental, and psychosocial needs while advocating for their rights and overall well-being
- Maintain ongoing communication with residents, families, physicians, hospitals, and the interdisciplinary care team
- Participate in care plan meetings and assist with resident-centered care planning, including MDS documentation as appropriate
- Coordinate or arrange counseling services, psychiatric services, and other community resources as needed
- Assist residents and families with financial, legal, and community resource referrals
- Develop and maintain professional relationships with local hospitals, physician offices, case managers, discharge planners, and other referral sources
- Assist with facility marketing initiatives, networking opportunities, and community outreach efforts to promote the services offered at JAG Healthcare Shelby
- Serve as a positive ambassador for JAG Healthcare by building relationships throughout the community and representing the organization professionally
- Maintain accurate and timely documentation while ensuring compliance with all applicable federal, state, and facility regulations
Skills & Qualifications
- Minimum of one (1) year of experience in long-term care social services, admissions, case management, healthcare marketing, or a related healthcare field preferred
- CNA certification preferred, but not required
- Knowledge of the aging process, including the physical, emotional, and psychosocial needs of older adults
- Understanding of resident rights and applicable federal and state long-term care regulations
- Excellent communication, organizational, and interpersonal skills
- Comfortable networking and building professional relationships within the healthcare community
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Positive, professional demeanor with excellent problem-solving abilities
- Previous MDS, admissions, discharge planning, or behavioral health experience is a plus
