Position Overview:
The Social Service Staff functions under the guidance and supervision of the Social Service Director to provide related social services which assist residents in maintaining or improving the ability to manage their physical, mental and psychosocial needs.
Assist with resident admissions and personal arrangements: Help with admission processes, facility tours, inquiries, and coordinate personal care needs (e.g., hair care, transportation, adaptive equipment, clothing).
Maintain communication and documentation: Keep contact with families and physicians, invite them to care plan meetings, and document all interactions and referrals in the social service log.
Support residents and families with forms and applications: Guide in completing advance directives, Medi‑Cal applications, voter registration, and other required documents.
Assist in resolving issues: Help address resident/family grievances, theft, and loss concerns as needed.
Participate in programs and meetings: Join behavior management plans, quality assurance programs, in‑service trainings, and staff meetings as directed.
Ensure compliance and confidentiality: Follow facility policies, laws, and regulations while maintaining confidentiality, integrity, and respect for residents’ rights.
Demonstrate professionalism and safety awareness: Act as a role model, maintain punctuality and workplace safety standards, and respond effectively in emergencies.
Qualifications:
Must possess, as a minimum, a bachelor’s degree in social work or a bachelor’s degree in a social service related field, psychology, human services, sociology, public health, counseling or mental health services.
Must have, as a minimum, one year of supervised social work experience, preferably in a skilled nursing setting or long-term care setting.
Familiarity with Medicare processes and regulations is a plus.
Ability to manage multiple priorities in a fast-paced environment.
Strong interpersonal skills with the ability to build rapport with diverse populations.
Excellent organizational skills and attention to detail are essential for maintaining accurate records.
