Assertive Community Treatment - Vocational Rehabilitation Qualified Professional (QP)

As the Voc Rehab QP on our Wake County ACT Team, you will be responsible for providing evidence-based, supported employment in a way that is consistent with the eight principles of IPS (Individual Placement and Support).
The primary goal is to educate the member about their opportunities and the benefits associated with work and/or school, assess for job readiness, prepare for employment and support throughout the process of seeking and maintaining employment to promote fulfilling productivity and community inclusion.
This team covers Wake County, NC - we provide mileage reimbursement!
Pay
$22.00 per hour.
Schedule
Monday - Friday, flexible.
Typical schedule is 830a-430p.

You will be eligible for medical benefits the first of the month following your first 30 days. Job Responsibilities of an ACT Team Vocational Rehabilitation Qualified Professional
- Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis.
- Establish relationships with employers and serve as liaison, advocate and coach for clients wishing to seek employment
- Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources.
- Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual’s needs.
- Participate in on-call crisis intervention rotation
- Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or Microsoft Teams.
- Documents participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes.
- Develop service plans and/or person-centered plans as appropriate, and screen referrals in coordination with the other Assertive Community Treatment Team members.
Qualifications of an ACT Team Vocational Rehabilitation Qualified Professional
Education and Experience
QP status:
- 4 year degree in Human Services + 2 years' experience with MH/SU population
OR
- 4 year degree non-Human Services + 4 years' experience with MH/SU population
- Must have valid North Carolina Driver License
- Minimum 1-year Experience with IPS model preferred
Knowledge, Skills, and Abilities
- Ability to easily connect with people with profound communication skills
- A constructive approach towards members
- Ability to maintain confidentiality of the information collected from the members and their families
- Ability to tackle complicated issues with ease
- Considerable knowledge of the dynamics and problems of addiction and current practices of treatment
- Strong commitment to the rights and abilities of each person with a severe and persistent mental illness to live in normal community residences
