Deliver skilled nursing care in accordance with physician orders and care plans

Assess, monitor, and document patients' physical and emotional health status

Identify changes in patient conditions and communicate findings appropriately

Develop and implement individualized care interventions

Collaborate with patients, families, and healthcare teams to ensure quality care
-
Maintain accurate and timely clinical documentation
-
Current and unrestricted LPN license in Wisconsin
-
Current BLS/CPR certification
-
Current TB test or Chest X-Ray
-
Previous nursing experience preferred (1 year or more)

Strong clinical, communication, and documentation skills
