We are seeking a dedicated and energetic Home Health Office Registered Nurse (RN) to join our dynamic healthcare team. Responsibilities
- Coordinate patient care plans by collaborating with interdisciplinary teams, including physicians, therapists, and social workers
- Manage patient documentation using electronic health record (EHR) systems such as Perfect Noteefied to ensure accurate and timely medical records
- Conduct comprehensive patient assessments, including vital signs, medication reconciliation, and physical examinations
- Knowledgeable in medical procedures such as IV infusions, catheter care and sterile technique adherence in accordance with best practices
- Knowledge of medication administration, disease management—including infectious disease care—and health coaching strategies to promote self-care
Pay: $36.00 - $50.00 per hour
