Frequent Users Service Engagement (FUSE) Program

Position Summary:
The Clinical Case Manager Lead (CCM Lead) is responsible for supervising and supporting staff while providing direct clinical case management services to families who frequently cycle through shelters. The CCM Lead oversees program coordination, ensures quality service delivery, supports family shelter partnerships and crisis response, and helps strengthen family stability and housing outcomes. Reports to the Director of Programs.
Position Responsibilities
Supervision and Staff Development
- Provide supervision and guidance to the FUSE team Case Managers
- Support high quality and compliant case documentation
- Offer coaching, training and assistance on best practices
- Assist staff with complex cases and systems navigation
Program Administration
- Ensure agency documentation is accurate and timely
- Coordinate communication and referral pathways with shelter partner
- Support program reporting and quality improvement efforts
- Provide training and technical assistance to shelter partners to support FUSE clients
Program Delivery and Client Services
- Maintain a small clinical caseload
- Conduct holistic assessments and service planning
- Provide crisis intervention and stabilization supports
- Coordinate linkage to housing, healthcare, benefits and family supports
Qualifications
- Master’s degree in Social Work, Counseling or related clinical field required
- LCSW or LCPC strongly preferred; LSW or LPC considered with strong clinical experience
- Experience supervising or clinically leading staff preferred
- Knowledge of homelessness, family systems, trauma-informed care and harm reduction
- Proficiency with Google Suite and electronic case management systems
Compensation and Benefits
Up to $85,000 annual salary, commensurate with experience.

Working Conditions
Travel required between agency sites, shelters and client residences. Sitting, standing and stair climbing required.

Pay: $75,000.00 - $85,000.00 per year
