Miracle Makers Home Care is seeking a Care Success Coordinator (CSC) to manage and support an assigned caseload of approximately 100 active clients and their caregivers.

What You’ll Do
- Serve as the primary point of contact for approximately 100 active clients and their caregivers.
- Build strong relationships with clients, families, and Direct Care Workers.
- Resolve client and caregiver concerns quickly and professionally.
- Conduct quarterly client and caregiver satisfaction surveys and routine wellness check-ins.
- Coordinate schedule changes, caregiver replacements, and service updates.
- Review and resolve HHAeXchange EVV exceptions, missed visits, and visit verification discrepancies.
- Process payroll-related corrections, timekeeping adjustments, and payroll inquiries.
- Monitor attendance, call-offs, and service continuity.
- Collaborate with Scheduling, HR, Payroll, Billing, and Operations to resolve issues efficiently.
- Maintain accurate documentation and ensure timely follow-up until concerns are fully resolved.
- Help improve client satisfaction, caregiver retention, and overall service quality.
Qualifications
- Excellent communication and customer service skills.
- Strong organizational, multitasking, and problem-solving abilities.
- Ability to work independently in a fast-paced environment.
- Healthcare, home care, customer service, scheduling, payroll, or case management experience preferred.
- Experience with HHAeXchange or another EVV platform is a plus.

Pay: $15.00 - $21.00 per hour
