RESPONSIBILITIES:
- Schedule medical appointments
- Transport, accompany and support individuals in ICF-MR and PLF programs to all medical appointments
- Prepare all paperwork related to medical appointments, including lab results, referrals and insurance documents
- Knowledge of purpose of medical appointments
- Knowledge of individual Behavior Plans to assist and support individual during medical appointment
- General knowledge of individual’s lifetime medical history to communicate to health care provider
- Documentation of follow-up appointments and re-calls
- Inform Nursing staff of medical changes, new orders and results of medical appointments
- Prepare weekly medical appointment schedule and distribute to appropriate parties
- File all medical, nursing and health care documents as directed
- Maintain confidentiality of individual’s medical records and personal information
- Complete 24 hours of in-service annually
- Ensures compliance with regulatory standards as they relate to specific program areas, such as licensing regulations and current policy
- Consistently demonstrates good judgment and decisions regarding health concerns
- Creates a working climate that provides growth and job satisfaction of all personnel
- Demonstrate an ability to be flexible and deviate from the routine, according to the needs of the program
- Will perform other duties as directed by Administrator, Program Director and Director of Residential Operations
- Maintain a clean and professional appearance
- Report for duty on time as scheduled

QUALIFICATIONS:
- High School graduate with medical and developmental disabilities experience preferred
- Must be able to read, write and speak the English language
- Current PA driver’s license
- Clearances: Physical, PA. State Police, Child Abuse and FBI (if applicable)
- Physical ability for prolonged standing, lifting and walking. No lifting restrictions.
- Basic computer skills
WORKING CONDITIONS:
- Ability to work in a variety of settings and environments, moderate exposure to adverse weather conditions
- Fixed hours with occasional flexible hours
