Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary
- Monitor and document patient health status by taking vital signs and observing for any changes in condition or behavior
- Administer medications accurately following medical instructions while maintaining strict adherence to HIPAA regulations for patient privacy
- Support patients with specialized needs such as catheter care, dementia and Alzheimer’s care, hospice support, and memory care routines
- Maintain detailed medical documentation within Electronic Health Record (EHR) systems and ensure all records are accurate and up-to-date
- Prepare nutritious meals tailored to individual dietary requirements and assist with meal planning and preparation
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being
- Collaborate effectively with nursing staff and other healthcare professionals to deliver comprehensive patient care in home or community settings
Requirements
- Proven experience in home health, assisted living, nursing homes, or long-term care environments
- Strong knowledge of medical terminology, medication administration, vital signs monitoring, and medical documentation practices
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia care are preferred
- Ability to work independently while managing multiple patient needs with compassion and professionalism
- Experience working with individuals with developmental disabilities or disabilities in general is highly valued
- Familiarity with EMR/ EHR systems for documentation purposes
- Valid driver’s license and reliable transportation are often necessary for travel between patient locations
- Commitment to maintaining patient privacy and confidentiality in accordance with HIPAA standards
