Licensed Physical Therapist - In-Home Visits, Chicago North Side
Elliott Concierge Physical Therapy is hiring a licensed Physical Therapist to provide premium, one-on-one in-home physical therapy in select Chicago neighborhoods.
This is a flexible, part-time 1099 role for a therapist who wants strong per-visit pay, clinical autonomy, high-quality patient relationships, and the ability to provide thoughtful care without the pace and volume of a traditional clinic.
Service Area
This role is focused on in-home visits in select Chicago neighborhoods, especially:
- Lincoln Park
- Lakeview
- Old Town
- Gold Coast
- Streeterville
- River North
- West Loop
- North Center
- Roscoe Village
- Bucktown
- Wicker Park
- Nearby city neighborhoods
Applicants should be realistically willing to serve several of the neighborhoods listed above.
At this time, this opening is not primarily focused on southwest suburbs, western suburbs, far north suburbs, or general Chicagoland coverage.
Compensation and Schedule
Compensation is typically $100-$125 per completed visit, depending on experience, visit type, geography, and clinical fit.
Higher compensation may be available for select evening or weekend visits, short-notice visits, or visits in more difficult-to-cover areas.
Expected volume is approximately 6-20 hours per week, with opportunity to grow as patient volume expands.
Therapists provide their availability, identify the Chicago neighborhoods they are comfortable serving, and accept visits that fit their schedule and service area.

ECPT handles patient acquisition, intake, payment logistics, and general practice support so the therapist can focus primarily on patient care, communication, and the patient relationship.

- Are primarily looking to serve the suburbs rather than the listed Chicago neighborhoods
Responsibilities
- Provide one-on-one physical therapy visits in the patient’s home
- Evaluate and treat patients based on clinical need and professional judgment
- Develop individualized plans of care
- Provide clear education to patients and families
- Progress exercise, mobility, strength, balance, gait, and functional training
- Communicate professionally before and after accepted visits
- Complete documentation in a timely manner
- Represent Elliott Concierge Physical Therapy with professionalism, warmth, and reliability

Requirements
- Active Illinois Physical Therapist license
- Doctor of Physical Therapy or equivalent physical therapy degree
- Professional liability insurance
- Reliable transportation
- Comfortable providing care in patients’ homes
- Strong communication and documentation habits
- Prior outpatient orthopedic, home health, geriatric, neurologic, or post-surgical experience preferred

Pay: $100.00 - $125.00 per hour
