INDEPENDENT LIVING COORDINATOR/ RAMP PROGRAM COORDINATOR

General Responsibilities
Provide independent living skills training, peer counseling, and support to individuals who have disabilities in the environment they require. Assist individuals in their effort to return to the community from institutions and to stay out of institutional living. Conduct presentations and conduct and participate in individual and systemic advocacy activities.
Responsible for managing the ramp construction program. Must have a valid driver's license. Must be able to lift 50 pounds. Responsible for facilitating ramp construction which includes loading and unloading trailer, operating power tools, ability to read a tape measure, ensuring jobsite safety, and delegating tasks to volunteers.
Primary Duties include:
Conduct initial assessment to determine consumer's individual needs.
Assist the consumer in developing initial, intermediate and long range goals.
Provide independent living skills training.
Track and monitor consumer progress.
Identify individuals in institutions who want to return to the community.
Provide and coordinate services designed to help individuals in an institution return to the community and help individuals remain in the community.
Provide information and referral services.
Provide individual peer support.
Facilitate peer support groups among The WILL Center consumers.
Conduct public education and awareness activities.
Provide community organizations with training and information related to disability issues.
Activate, track and inactivate consumer files using The WILL Center's policies and procedures.
Maintain current and accurate consumer files.
Track community activities and I & R contacts.
Gather program outcomes based on consumer files and evaluation data.
Contribute to the planning of The WILL Center's strategic goals.
Receive and record consumer calls for ramps.
Maintain database of ramp requests.
Plan ramp construction schedule.
Coordinate with Independent Living Coordinators/Chapter 2 Program staff for other consumer services beyond the ramp.
Coordinate site mapping and ramp construction plans.
Recruit and support volunteers to conduct site for mapping and ramp construction.
Responsible for procuring materials to maintain prefab stock at ramp shop. This includes purchasing lumber and fasteners, delivering materials to prefab shop, and transporting prefabricated ramp sections back to shop.
Seek grant and private donations for materials/program support.
Other duties as assigned.
*This role is a 36 hour work week. Availability on some Saturdays is required during ramp builds.
Pay: From $43,296.00 per year
