Job Summary
We are seeking a dedicated Registered Nurse to join our team. The ideal candidate will have a passion for patient care and possess a diverse skill set to excel in bringing hope to our aging clients while empowering them to remain independent in their homes.
Responsibilities
- Provide direct patient care in various home care settings
- Administer medications and treatments as prescribed by physicians
- Monitor patient conditions and report changes to the medical team
- Educate patients and their families on health management and disease prevention
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
Skills
- Knowledge of sterile processing techniques
- Familiarity with internal medicine practices
- IV & Infusion Therapy experience
- Proficiency in patient care across different specialties such as PICU, geriatrics, and internal medicine
- Ability to administer injections with precision
- Strong understanding of anatomy and physiology
- Excellent communication and teaching skills
- Axxess EMR system experience
- Home Health experience (3 years preferred)
- Excellent documentation skills and familiarity with utilizing Axxess for SOC, ROC and Recertifications

- Care plan development and care coordination

Pay: $41.75 - $48.50 per hour
