Willow Park Health is a rapidly growing behavioral health practice dedicated to providing exceptional psychiatric and psychotherapy services to older adults living in assisted living and senior living communities throughout Long Island.
We are seeking a compassionate and motivated Licensed Therapist (LMSW, LCSW, or LMHC) to join our collaborative behavioral health team.
This is an ideal opportunity for a therapist who enjoys building meaningful relationships with patients while working alongside experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) in a supportive, low-stress environment.
What You'll Do
Provide individual psychotherapy to older adults in senior living communities.
Complete comprehensive behavioral health assessments.
Develop individualized treatment plans.
Collaborate closely with PMHNPs and primary care providers.
Document visits using our electronic medical record.
Participate in interdisciplinary care planning to improve patient outcomes.

Qualifications
Active New York license as:
LMSW
LCSW
LMHC
Excellent communication and interpersonal skills
Experience working with older adults or behavioral health preferred, but not required
Strong documentation and organizational skills
