- Attend biweekly Multidisciplinary Team (MDT) meetings (frequency may vary based on program needs).
- Document services, observations, and client interactions in CCS's electronic health record in accordance with agency policies and regulatory requirements.
- Support hybrid care workflows by coordinating remote psychiatric appointments for clients attending in-office sessions and facilitating communication between clients and psychiatric providers to ensure continuity of care.
Qualifications
- Current and unrestricted California Licensed Vocational Nurse (LVN) license.
- Experience working in behavioral health, mental health, psychiatry, or outpatient healthcare settings preferred.
- Strong communication, organizational, and documentation skills.
- Ability to work effectively with clients, psychiatrists, therapists, and multidisciplinary treatment teams.
- Proficiency with electronic health record (EHR) systems and basic computer applications.
Work Schedule

- Services provided at CCS Victorville and Crestline clinic locations, based on scheduling needs.
- Independent contractor position; hours may vary based on client demand and psychiatrist availability.
