Job Summary
We are seeking a compassionate and experienced Registered Nurse to join our growing team. This role is responsible for completing admissions and providing routine skilled nursing visits for home health patients.
The ideal candidate is clinically strong, organized, and comfortable working independently in the field while delivering high-quality, patient-centered care.
Responsibilities
- Complete home health admissions, including comprehensive assessments and documentation
- Perform routine skilled nursing visits for home health patients
- Develop and implement patient care plans in collaboration with the interdisciplinary team
- Provide patient and family education on diagnoses, medications, and care plans
- Ensure accurate, timely, and compliant documentation
- Coordinate care with physicians, case managers, and other healthcare professionals
- Adhere to all regulatory and agency standards (CMS, state, and company policies)
Qualifications
- Active Registered Nurse (RN) license in the state of [State]
- Minimum 1–2 years of nursing experience (home health preferred)
- Strong clinical assessment and critical thinking skills
- Experience with OASIS documentation preferred
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver’s license and reliable transportation
