Job Overview
We are seeking a dynamic and compassionate Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join our team at a leading Skilled Nursing Facility. Responsibilities
- Conduct comprehensive psychiatric assessments, including mental status examinations and medication reviews, to develop personalized treatment plans.
- Prescribe and manage psychotropic medications in accordance with collaborative practice agreements and DEA regulations.
- Collaborate closely with interdisciplinary teams—including physicians, therapists, social workers, and nursing staff—to coordinate comprehensive patient care.
- Document all clinical encounters accurately within Electronic Health Record (EHR) systems like Epic or eClinicalWorks, maintaining compliance with HIPAA standards.
- Educate patients and their families on mental health conditions, medication management, and wellness strategies to empower self-care.
Requirements
- Valid state licensure as a Nurse Practitioner with certification in Psychiatric-Mental Health (PMHNP).
- Demonstrated experience working in Skilled Nursing Facilities or long-term care settings with geriatric populations.
- Proficiency in EMR/EHR systems such as Epic or Athenahealth for documentation and patient management.
- Strong knowledge of psychopharmacology, including prescribing medications within scope of practice.
- Experience with behavioral health interventions, crisis management, and case coordination across multidisciplinary teams.
- Familiarity with medical procedures such as IV infusion, venipuncture, suturing, catheter care, and basic life support (BLS).
- Excellent communication skills to effectively collaborate with healthcare professionals and engage patients compassionately.

Pay: $170,000.00 - $180,000.00 per year
