Compensation: $85 Per-Visit (negotiable)
Job Summary
We are seeking a compassionate, skilled, and dependable Occupational Therapist (OT) to join our home health team as an independent contractor. The Occupational Therapist will provide skilled therapy services to patients in their homes, helping them improve functional independence, safety, and quality of life. This position offers a flexible schedule and compensation on a per-visit basis, allowing clinicians to manage their own availability while delivering exceptional patient-centered care.
Essential Duties and Responsibilities
Perform comprehensive occupational therapy evaluations in patients' homes.
Develop, implement, and update individualized treatment plans based on physician orders and patient goals.
Provide skilled occupational therapy interventions to improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, cognitive skills, coordination, balance, and home safety.
Evaluate the home environment and recommend adaptive equipment, durable medical equipment (DME), and environmental modifications to promote independence and reduce fall risk.
Educate patients and caregivers on home exercise programs, adaptive techniques, energy conservation, safety strategies, and disease management.
Monitor patient progress and modify treatment plans as clinically indicated.
Complete all required documentation accurately and within agency timeframes using the designated electronic medical record (EMR) system.
Communicate patient progress, concerns, and recommendations with physicians and members of the interdisciplinary care team.
Participate in case conferences and coordinate care with other healthcare professionals as needed.
Comply with all applicable federal, state, and professional practice standards, including Medicare regulations, HIPAA requirements, and agency policies.
Maintain professional licensure, certifications, and liability insurance required for independent practice.
Travel to patient homes within the assigned service area.
Qualifications
Graduate of an accredited Occupational Therapy program with a Master's or Doctoral degree.
Current, unrestricted Occupational Therapist license in the state of practice.
Minimum of one (1) year of clinical experience preferred; home health experience is strongly preferred.
Current CPR/BLS certification.
Valid driver's license, reliable transportation, and current automobile insurance.
Professional liability (malpractice) insurance.
Ability to work independently with minimal supervision.
Strong organizational, communication, and clinical decision-making skills.
Proficiency with electronic medical record (EMR) documentation.
Independent Contractor Expectations
Maintain an active professional license and all required certifications.
Maintain professional liability insurance and any other required business insurance.
Accept patient assignments based on availability.
Manage personal schedule while meeting documentation and visit deadlines.
Provide services in accordance with agency policies, physician orders, and applicable federal and state regulations.
Responsible for personal tax obligations and expenses associated with independent contractor status.
Physical Requirements
Ability to stand, walk, bend, kneel, lift, and assist patients with mobility and therapeutic activities.
Ability to safely perform patient transfers and therapeutic interventions.
Ability to travel throughout the assigned geographic service area.

Competitive per-visit compensation.
Flexible scheduling based on contractor availability and patient needs.
Mileage reimbursement, if applicable, will be provided according to agency policy.
Independent contractor (1099) status; no employee benefits are provided unless otherwise specified.
Preferred Skills
