Optimal Home Rehab LLC is a clinician-owned, non-traditional Physical, Occupational & Speech Therapy outpatient private practice serving aging adults throughout Delaware County and Philadelphia. Founded in 2018 by a physical therapist: our mission is simple: help older adults maintain their independence and quality of life — in the place they know best, their own home.
We’ve grown from a one-person mobile practice to a full clinical team, and we’re looking for a Physical Therapist who wants meaningful, one-on-one patient care without the volume pressure of a clinic setting.

What You’ll Do
- Deliver in-home physical therapy evaluations and treatment to aging adults in Delaware County
- Build genuine rapport and trust with patients and their families
- Complete timely, accurate documentation to support Medicare Part B compliance
- Work independently while staying connected to a collaborative clinical team
Requirements
- Current PA Physical Therapy license (or license-eligible for new grads)
- Current CPR certification
- Reliable transportation and valid driver’s license
- Comfortable working independently in a per-visit, contractor model
