What You'll Be Doing
You'll serve as one of the clinical communication hubs for our practice, helping ensure patients receive timely, coordinated care behind the scenes.
Your day will include triaging incoming calls, reviewing patient concerns, collaborating with providers, coordinating communication with senior living communities, processing medication changes and provider orders, documenting accurately within the EMR, and ensuring important clinical information reaches the right people quickly.
You'll also help reconcile patient records, respond to clinical messages, support quality improvement initiatives, and keep care moving efficiently across our growing practice.

We're Looking For Someone Who...
Has an active Minnesota Licensed Practical Nurse (LPN) license and enjoys thinking critically through clinical situations.
Communicates confidently and professionally with patients, families, providers, and healthcare partners.
Thrives in a fast-paced office environment where organization, accuracy, and multitasking are essential.
Has previous experience in triage, clinic nursing, primary care, geriatric care, care coordination, or another outpatient healthcare setting. Experience with PointClickCare, Eldermark, or Google Workspace is helpful but not required.
Most importantly, we're looking for someone who enjoys helping patients by keeping healthcare organized, coordinated, and moving forward.
