Onyx Home Care is seeking a compassionate, motivated, and dependable Certified Occupational Therapy Assistant (COTA) to join our skilled home health team. This position provides an opportunity to work one-on-one with patients in the comfort of their homes while helping them regain independence, improve functional abilities, and achieve meaningful goals.

Onyx Home Care provides skilled home health services throughout Volusia, Flagler, and St. John’s Counties. The Certified Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to provide occupational therapy interventions in accordance with the established plan of care.
The COTA will provide individualized treatment to patients in their homes, focusing on improving functional independence, safety, mobility, activities of daily living, and quality of life.
This position is ideal for a COTA who enjoys building meaningful relationships with patients and families and wants the flexibility and autonomy of providing therapy in a home health setting.

Provide occupational therapy treatment and interventions under the direction and supervision of the Occupational Therapist.

Follow the established plan of care and treatment goals for each patient.

Assist patients in improving their ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

Provide therapeutic activities and exercises designed to improve strength, coordination, balance, endurance, and functional performance.

Assist patients with developing strategies to maximize independence and safety within the home.

Provide instruction and education to patients and caregivers regarding therapeutic techniques, safety, adaptive equipment, and home programs.

Identify potential safety concerns within the patient's home environment and communicate recommendations to the supervising Occupational Therapist.

Assist with functional mobility and transfers as appropriate to the patient's plan of care.

Provide training related to adaptive equipment and compensatory techniques as directed by the Occupational Therapist.

Monitor patient response to treatment and communicate changes in condition, progress, or concerns to the supervising Occupational Therapist and appropriate members of the healthcare team.

Maintain accurate, timely, and complete clinical documentation in the agency's EMR.

Complete required visit documentation within agency and regulatory timeframes.

Communicate effectively with patients, caregivers, physicians, nurses, therapists, and other members of the interdisciplinary team.

Participate in care coordination and contribute to the overall plan of care.

Maintain patient confidentiality and comply with HIPAA and all applicable federal and state regulations.

Follow all Onyx Home Care policies, procedures, clinical standards, and quality-assurance requirements.

Maintain professional boundaries and demonstrate compassion, respect, and professionalism during every patient interaction.

Perform other duties as assigned.

Current certification as a Certified Occupational Therapy Assistant (COTA).

Graduate of an accredited Occupational Therapy Assistant program.

Current Florida COTA license/registration in good standing.

Current CPR certification or ability to obtain certification as required by the agency.

Previous home health experience is preferred but not required.

Strong communication, organization, and time-management skills.

Ability to work independently while maintaining effective communication with the supervising Occupational Therapist and clinical team.

Demonstrated ability to establish rapport with patients and families.

Reliable transportation and the ability to travel throughout the assigned service area.

Valid driver's license and automobile insurance.

Ability to complete required documentation accurately and within established timeframes.
The COTA must be able to perform the essential functions of the position, including:

Frequent standing, walking, bending, reaching, and kneeling.

Ability to assist patients with transfers and functional mobility as appropriate.

Ability to demonstrate therapeutic activities and exercises.

Ability to safely navigate a variety of home environments.

Ability to lift and/or move equipment and supplies as required for patient care.
