Sojourner House, Inc. is a homeless shelter for families located in Roxbury. Families are referred through the Executive Office of Housing and Livable Communities (EOHLC). At full capacity Sojourner House can provide shelter for up to nine families. After families secure permanent housing, Sojourner House continues to offer stabilization services for up to two years.
Position Summary:The Case Manager plays a vital role in supporting families experiencing homelessness. This role requires a proactive professional who can take initiative, communicate clearly and make sound decisions. The Case Manager is responsible for assessing and addressing barriers to housing, coordinating supportive services, and ensuring compliance with shelter policies. The ideal candidate is highly organized, able to make sound decisions, and committed to empowering families as they transition toward stability and permanent housing. The Case Manager works closely with the Housing Advocate and is supervised by the Operations Manager.
Duties and Responsibilities
· Provide intensive case management to families facing homelessness.
· Communicate effectively with families, colleagues, and external partners to ensure clarity and follow-through.
· Identify, coordinate, and connect families with appropriate community-based resources.
· Ensure families comply with Sojourner House and EOHLC Uniform Shelter Rules.
· Conduct intakes, maintain accurate case notes, develop comprehensive Rehousing Plans and Assessments.
· Consistently hold families accountable and when necessary, complete non-compliance submissions accurately and in a timely manner.
· Collaborate closely with the Housing Advocate.
· Network with homeless providers and community-based organizations to enhance support for families.
· Coordinate front office coverage, maintain clear and consistent communication while supervising and supporting Shelter Support Staff.
· An understanding of the complexities and challenges of securing housing.
·Facilitate monthly house meetings and coordinate family workshops and training.
· Strong attention to detail, follow-through, and the ability to interpret information and draw conclusions.
· Demonstrates the ability to work independently while also collaborating effectively as part of a team.
· Work one required evening shift per week (Tuesdays, 11am-7pm ).
· Possess a valid Massachusetts driver’s license and a reliable vehicle.
Required Experience and Skills
· Prior case management experience.
· Experience providing guidance or oversight to staff.
· Proficient in MS Word, Google Workspace, and the ability to learn Efforts to Outcome (ETO) software.
· Demonstrates strong organizational skills and the ability to manage multiple tasks effectively.
· Exceptional written and verbal communication skills, including the ability to compose professional emails, notices, and reports.
· The ability to take initiative, solve problems proactively and work independently when necessary.
· Proven ability to work collaboratively as a team player.
· Bilingual in Haitian Creole or Spanish a plus.
Physical Requirements: Standing, sitting, bending, climbing, carrying, walking, and driving.

Pay: $50,000.00 - $55,000.00 per year
