Job Summary
We are seeking a dedicated and energetic Home Health Physical Therapist to join our dynamic healthcare team. In this vital role, you will deliver personalized physiotherapy services directly in patients’ homes, promoting recovery, independence, and improved quality of life. Your expertise will help patients across diverse age groups and health conditions regain mobility, manage pain, and achieve their rehabilitation goals. This position offers the opportunity to make a meaningful difference by providing compassionate care in the comfort of patients’ familiar environments. Home Health Physical Therapist provides services indicated for functional limitations and deficits in safety, mobility, strength, and range of motion. Services provided by the physical therapist shall be performed within the scope of practice authorized by the license issued by the State for the practice of physical therapist.
Responsibilities
- Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop tailored care plans that address individual needs.
- Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics interventions, and geriatrics-focused exercises to facilitate recovery.
- Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from inpatient or outpatient settings to home care.
- Deliver hands-on patient care, including therapeutic exercises, manual therapy, and mobility training, while monitoring progress and adjusting treatment plans accordingly.
- Educate patients and their families on injury prevention, pain management strategies, and home exercise programs to foster ongoing health improvements.
- Maintain accurate documentation of patient interactions, treatment progress, and care plans using electronic medical records (EMR) systems.
- Collaborate with healthcare providers in acute care hospitals, clinics, outpatient centers, and community settings to optimize patient outcomes.
- Participate in case conferences and team meetings to discuss patient progress and modify care strategies as needed.
- Support patients with complex needs by integrating discharge planning processes that align with their long-term health goals.
- Stay current with advancements in physiotherapy practices and healthcare regulations to ensure the highest standard of patient care.
Requirements
- He/she must be a graduate of an approved school for Physical Therapist.
- He/she must be currently licensed and registered in this state.
- He/she must have at least one year of experience in Physical Therapy.
- Strong knowledge of physiology, anatomy, medical terminology, and various physiotherapy techniques including trigger point therapy and orthopedics.
- Demonstrated ability to develop comprehensive care plans tailored to diverse patient populations.
- Experience with EMR systems for documentation and patient record management.
- Familiarity with discharge planning processes for inpatient and outpatient settings.
- Excellent patient care skills with a compassionate approach that fosters trust and engagement.
- Ability to work independently in patients’ homes while maintaining professional standards of safety and hygiene.
- Strong communication skills for effective collaboration with multidisciplinary teams and educating patients’ families.

Pay: $80.00 - $100.00 per hour
