American Traveler is seeking a Physical Therapist for a Home Health assignment on Kaua'i, requiring 1+ years of home health experience and HI licensure.

Home health setting serving the Kaua'i community

16-week assignment with day shift schedule (5x8 hours, 8:00 AM - 4:30 PM)
-
Occasional floating to cover Oahu, Maui, and/or Big Island may be required with all float expenses paid
-
Uses HomeCare HomeBase (HCHB) for documentation

Productivity requirement of 25 units per week

Patient population includes geriatric, neurological, orthopedic, cardiopulmonary, and post-surgical cases

Current BLS certification (AHA) required

HI state license required (license required if applicable)

Minimum 1 year of home health experience required
-
Minimum 3 months of OASIS documentation experience required
-
Experience with start of care and OASIS assessments required
-
Skilled in gait training, adaptive/assistive devices, and mobility equipment (walkers, wheelchairs, Hoyer lifts, crutches)
-
Experience with cardiopulmonary conditions (COPD, CHF, CABG, MI, PVD) and orthopedic conditions (total hip/knee replacement, post-op spinal surgery)

Experience with neurological and cognitive conditions (stroke, Parkinson's disease, dementia/Alzheimer's)

Strong skills in patient/family education, home exercise programs, and safety/injury prevention

Wound care, acute care, and lymphedema management experience preferred

Experience with Multiple Sclerosis, muscular dystrophy, NDT, spinal cord injury, and traumatic brain injury preferred

Provide in-home physical therapy services to elderly (kupuna) patients across Kaua'i
-
Responsibilities include evaluations, reassessments, care planning, and discharge planning using OASIS documentation

Scrubs in any color and closed-toed shoes required for this role
