About the Role:
As a Physical Therapist (PT) at Astoria Skilled Nursing and Rehabilitation, you will have the opportunity to make a positive impact on the lives of our guests. You will work closely with a multidisciplinary team to develop and implement individualized treatment plans designed to promote recovery, restore function, and improve overall well-being. Our rehab-recovery suites are equipped with the latest technology and equipment, providing a comfortable and inviting environment for our guests.
Responsibilities:
Evaluate and assess patients' physical impairments, functional limitations, and disabilities
Develop and implement individualized treatment plans to achieve patients' goals and optimize outcomes
Provide physical therapy interventions, including exercises, modalities, and other therapies
Collaborate with other healthcare professionals to ensure a comprehensive approach to patient care
Conduct patient education and training to ensure patients and families understand the treatment plan and how to maintain progress after discharge
Document patient progress, treatment plans, and outcomes in accordance with company policies and regulatory requirements
Ideal Candidate:
Bachelor's or Master's degree in Physical Therapy
Current licensure as a Physical Therapist (PT) in the state of Ohio
