We are seeking a Licensed Professional Counselor Mental Health (LPC-MH / LPCM) or Licensed Professional Counselor (LPC) to join our team as a Psychiatric Case Manager. This role is ideal for a compassionate mental health professional who enjoys working directly with patients, coordinating services, and helping individuals navigate their behavioral health journey.
Job Summary
The Licensed Psychiatric Case Manager will provide behavioral health support, care coordination, patient advocacy, and case management services for individuals with mental health needs. The position works closely with providers, community resources, and healthcare teams to ensure patients receive appropriate treatment and support.
Responsibilities
- Conduct behavioral health assessments and psychosocial evaluations
- Provide individual support, counseling, and care coordination services
- Develop and monitor individualized care plans based on patient needs
- Coordinate referrals to mental health, medical, and community resources
- Assist patients with accessing available healthcare and social services
- Collaborate with psychiatrists, medical providers, therapists, and care teams
- Maintain accurate and timely documentation of patient interactions and services provided
- Follow confidentiality standards and HIPAA regulations
- Provide ongoing patient support and follow-up to improve outcomes
Qualifications
- Active LPC, LPC-MH, LPC Associate, or equivalent Texas behavioral health license
- Master’s degree in Counseling, Psychology, Social Work, or related field
- Experience in behavioral health, case management, counseling, or community mental health preferred
- Strong communication and organizational skills
- Ability to work independently while collaborating with a multidisciplinary team
- Compassionate, patient-focused approach
Preferred Qualifications
- Experience working with psychiatric patients or underserved populations
- Knowledge of community mental health resources
- Experience with electronic medical records (EMR) documentation

Pay: $25.00 - $30.00 per hour
