Job Summary:
We are seeking a highly skilled and compassionate Home Care Team Leader to join our team in Bay Minette, Alabama. As a Home Care Team Leader, you will be responsible for overseeing and leading a team of home care professionals to provide exceptional care and support to our clients. Responsibilities:
Lead a team of home care professionals to provide medical and non-medical services to clients in their homes
Conduct initial assessments and develop individualized care plans for clients
Supervise and evaluate team members to ensure compliance with company policies and procedures
Collaborate with healthcare professionals, clients, and families to ensure continuity of care
Provide education and training to team members on new procedures, protocols, and best practices
Maintain accurate records and documentation of client care and team member performance
Requirements:
Bachelor's Degree in Nursing, Health Care Administration, or related field
Minimum 2+ years of experience in home care, hospice, or healthcare management
Strong leadership and communication skills
Ability to work independently and make sound decisions
Proficiency in electronic medical records and other healthcare software
Current CPR certification
Knowledge of Medicare and other payor sources
Ability to pass a criminal background check
