Responsibilities
- Deliver comprehensive nursing care to patients in their homes, including medication administration, wound care, and vital sign monitoring.
- Conduct thorough patient assessments to evaluate health status and develop individualized care plans aligned with physician orders.
- Manage complex medical procedures such as IV infusion, catheterization, tube feeding, and sterile dressing changes using aseptic techniques.
- Document all patient interactions accurately within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Educate patients and their families on disease management, medication safety, and health promotion strategies tailored to conditions such as Alzheimer’s disease, dementia, or chronic pain.
- Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to specialists or community resources.
- Perform specialized skills including phlebotomy, injection administration, specimen collection/processing, and respiratory support such as ventilator management when needed.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN).
- Proven experience in home health nursing or related settings such as long-term care facilities, nursing homes, assisted living communities, or hospitals.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and clinical procedures including IV insertion and catheter care.
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
- Experience working with diverse patient populations including individuals with developmental disabilities, Alzheimer’s/dementia care, behavioral health needs or acute/chronic conditions.
- Ability to perform physical examinations and assessments confidently while maintaining infection control standards.
- Excellent communication skills for triage calls and patient education; ability to work independently with minimal supervision.
- Certifications in Basic Life Support (BLS) and other relevant credentials are preferred; additional experience in ICU, emergency medicine or trauma centers is advantageous.
