Job Summary
We are seeking a dedicated and knowledgeable Staff Pharmacist to join our team. The ideal candidate will have a strong background in clinical pharmacy and patient care, with experience in various pharmacy settings including LTC (Long-Term Care) and acute care environments. This role involves providing comprehensive pharmaceutical care, managing medication therapy, and ensuring the safe and effective use of medications for patients.
Responsibilities
- Provide expert pharmaceutical care to patients, ensuring optimal medication management and patient safety.
- Conduct patient assessments to evaluate medication needs and outcomes.
- Collaborate with healthcare providers to develop and implement individualized medication plans..
- Administer immunizations and manage patient education regarding medications and health conditions.
- Maintain accurate inventory management of pharmaceuticals and supplies.
- Utilize pharmacy software, EMR systems, and EHR systems for efficient documentation and patient record management.
- Participate in pain management strategies and critical care initiatives as needed.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Provide guidance and training to pharmacy technicians as necessary.
Requirements
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Active pharmacist license in the state of practice.
- Strong clinical pharmacy experience with knowledge of medical terminology, anatomy, physiology, and pharmacology.
- Excellent patient service skills with a focus on patient care and assessment.
- Familiarity with compounding medications and aseptic techniques is essential.
- Ability to manage inventory effectively while ensuring compliance with regulatory standards.
