As an LPN Intake Coordinator, you’ll work on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.
To receive, evaluate, facilitate, and manage inquiries and referrals for agency services from all sources and facilitate the transition of patients from an on-site unit of a hospital or skilled nursing facility to the home or hospice house.
Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on company's services available in a market.

Ensure effective communication and collaboration with program staff and other field sales resources via weekly meetings. Actively participate in weekly program business development meetings, bringing relevant data, reports, as well as information regarding changes within accounts and referral sources.
Provide education to facility staff regarding home care and hospice.
Must be registered to practice in NYS as a licensed practical nurse
One year experience in acute care setting
Experience in a certified home health agency
