Your Office Is the Community
One day may take you into a memory care community in Owatonna.
Another may include patients around Rochester.
Another may bring you farther south toward Albert Lea.
Wherever you're rounding, your purpose remains the same: bring proactive medical care directly to seniors who benefit from having a consistent provider where they live.
You'll work alongside nurses, caregivers, therapists, pharmacists, families, and facility leadership to understand the complete picture surrounding each patient.
That can mean catching a subtle change before it becomes an emergency, helping stabilize a chronic condition, or making sure a hospital discharge plan actually works once the patient returns home.
Your work helps strengthen continuity of care, reduce unnecessary hospitalizations, and keep older adults healthier where they call home.
What You'll Be Doing
- Provide comprehensive primary care throughout senior living and long-term care communities.
- Travel throughout Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities.
- Diagnose and manage acute and chronic medical conditions.
- Perform comprehensive assessments, preventative care, and routine follow-up visits.
- Develop individualized treatment plans.
- Manage chronic disease and monitor changes in condition.
- Coordinate care following hospitalizations, rehabilitation stays, and specialist visits.
- Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and senior living teams.
- Educate patients and families regarding diagnoses, medications, and treatment plans.
- Help improve outcomes through proactive and preventative medicine.
- Maintain accurate and timely clinical documentation.

You don't need to come directly from senior living. We're especially interested in NPs who genuinely enjoy caring for older adults and want the opportunity to build longer-term patient relationships.
What We're Looking For
Required Qualifications
- Active Minnesota Nurse Practitioner license.
- Excellent assessment, diagnostic, and clinical decision-making skills.
- Ability to practice independently while collaborating with physicians.
- Strong communication and care coordination skills.
- Passion for serving older adults.
- Comfortable managing medically complex patients.
- Reliable transportation and willingness to travel throughout Southern Minnesota.
Preferred Qualifications
- Experience caring for medically complex older adults.
- Chronic disease management expertise.
- Community-based or senior care experience.
- Assisted living, memory care, or skilled nursing experience.
- Advanced wound care experience.
- Wound Care Certification (WCC) is a plus.
